Enrollment Forms
Shortly after an offer of enrollment is made, we are required to collect enrollment documents pursuant to state and federal law. Links to the required documents are below; or, you may download our Enrollment Packet.
Documents Required for Enrollment
All of the following are required for enrollment, none of which are used to make enrollment decisions.
Proof of Arizona Residency
If the student is homeless, these forms do not apply. Submit one of the following:
A letter from an authorized representative of the agency having custody
Proof of Age and Identity
Submit one of the following:
A certified copy of the student’s birth certificate.
Reliable proof of the student’s identity and age, such as a baptismal certificate, an application for a social security number, or original school registration records and an affidavit explaining the inability to provide a copy of the birth certificate.
A letter from the authorized representative of an agency having custody of the student pursuant to ARS Title 8, Chapter 2 certifying that the student has been placed in the custody of the agency as prescribed by law.

Submit the Military Student Identifier Form:
Documents Required for Attendance
The following are not a condition for enrollment, but are to be received prior to attendance.
Immunization Record
Submit current immunization records or exemption form. Immunization records may be submitted by the parent, legal guardian, or obtained from the student’s prior school.
Academic Records
Submit all of the following that are applicable; this information is often provided by the student’s previous school.
Withdrawal form from previous school
IEP/504 information, if applicable
Official high school transcript, if applicable
